Provides vocational rehabilitation services for eligible injured workers. Applications can be obtained from the hospital or doctor's office where the initial treatment was provided. These applications must be submitted within one year from the date of the injury.
Intake Process: You can either call or visit in person to register.
Fees: There are no charges.
