This program delivers vocational rehabilitation services to eligible workers who have sustained injuries. Applicants should obtain their application from the hospital or doctor's office where they first received treatment. Applications need to be submitted within one year from the date of the injury.
Intake Procedure: Interested individuals can either call or visit in person to enroll.
Fees: There are no associated costs.
