About this resource
This initiative delivers extensive wraparound care and intensive management to aid community members in fulfilling vital life needs and enhancing health outcomes. It connects individuals to housing navigation, retention, and ongoing support services. Additionally, it provides help in establishing primary care medical homes, coordinating medical transportation, and accessing community resources.

Who can use this
- To qualify, applicants must be 18 years or older, reside in Santa Cruz County, and have Medi-Cal through Central California Alliance for Health (CCAH). Eligible groups include adults or families facing homelessness, those with Serious Mental Illness (SMI) or Substance Use Disorder (SUD), frequent users of healthcare services, individuals at risk for long-term care, nursing facility residents transitioning to the community, those with intellectual/developmental disabilities, pregnant and postpartum individuals, and adults re-entering society after incarceration.
