This initiative offers housing navigation, personalized case management, and resource connections for young adults aged 18-24 who have previously been in the foster care system. It includes life skills training aimed at helping participants secure and maintain stable housing, as well as mentorship and customized transition planning to promote long-term independence.
Intake Procedure: Interested individuals can apply by calling or emailing.
Fees: There are no associated fees.
Who can use this
- Young adults (ages 18-24) who have exited foster care or probation services after turning 16 and are at imminent risk of or currently facing homelessness.
