This program offers thorough care management and coordination of various health services, including physical, mental, and dental care, through a designated care manager for eligible Medi-Cal members. It also provides assistance with housing resources, transportation, food aid, and community programs.

- Eligibility requires being a Medi-Cal member, with qualifying life situations such as homelessness risk, frequent hospital visits, serious mental health or substance use issues, complex medical conditions, pregnancy or postpartum status, risk of institutionalization, or involvement in foster care.
