About this resource
This service provides adult day health care designed to enhance independence and avert institutionalization. It includes health monitoring, care coordination, family support, therapeutic activities, as well as physical, occupational, and speech therapies.
Intake Procedure: A referral from a primary care physician is required (Elderday can assist in obtaining this after the initial contact).
Fees: A sliding scale is available, and the program accepts Medi-Cal, Medicare, and private insurance.
Who can use this
- Eligible participants include seniors or adults with disabilities facing complex medical issues like head injuries, chronic mental health challenges, or dementia.
