About this resource
This service offers a wide range of dental care, such as cleanings, oral hygiene education, exams, x-rays, sealants, fillings, extractions, root canals, dentures, crowns, and operates a mobile dental van along with the Smiles Dental Project for school dental care.
Intake Procedure: To make an appointment, please call or email.
Who can use this
- Individuals residing in the service areas are eligible, with a focus on those who are medically underserved and regardless of their ability to pay.
Documents you may need
- If the client is applying for the sliding scale, they must provide the following: Completed application Photo ID Proof of insurance Proof of income Birth certificate Social security card Proof of residency
