This service offers guidance and support for applying to a variety of public benefits such as Connect for Health and OmniSalud (Colorado's health insurance marketplaces), Medicaid, Medicare, Social Security Disability (SSI), Social Security Disability Insurance (SSDI), Supplemental Nutrition Assistance Program (SNAP), and the Colorado Low-income Energy Assistance Program (LEAP). Additionally, individuals can call to complete an intake and set up an appointment. Referrals to nearby organizations offering further benefits and services are also available.

Documents you may need
- Must provide: A valid ID (driver's license, social security card, passport) Proof of legal residency (birth certificate, naturalization card) Proof of income, if applicable (pay stubs, W-2's, SSI or disability payments) Proof of assets, if applicable (bank statements) Proof of household expenses, if applicable (rent/mortgage bills, phone bill, child support payments)
