About this resource
The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is designed to support the health of income-eligible pregnant, postpartum, and breastfeeding women, along with infants and children under the age of 5. The program provides an eWIC debit card for purchasing nutritious foods, personalized nutrition guidance, referrals to community resources, and breastfeeding assistance, which includes access to breast pumps, lactation specialists, peer counseling, and support groups.
To apply, individuals should visit coloradowic.gov/find-wic-clinic to schedule an appointment at their nearest clinic. Alternatively, they can fill out an online referral form at healthinformatics.dphe.state.co.us/WICSignUp. A WIC representative will reach out within 10 days to determine eligibility and arrange an appointment.

- To qualify, individuals must reside in Colorado, adhere to specified income guidelines, and fall into one of the following categories: currently pregnant, postpartum, or breastfeeding.
Documents you may need
- Must provide: Identification for all family members applying for WIC Proof of address Proof of participation in Medicaid, SNAP, TANF, or FDPIR (Food Distribution Program on Indian Reservations), or proof of income (e.g. paystub, tax return, letter from employer) Baby's growth measurements (height and weight) and hemoglobin levels. Growth measurements must be within the last 60 days.
