Medicaid - Idaho Home Choice

The Idaho Home Choice Money Follows the Person Demonstration Grant aids individuals in transitioning from institutional care, such as skilled nursing facilities, intermediate care facilities, or mental health institutions, to community living in settings like apartments, private homes, or certified family homes and residential assisted living facilities. The MFP initiatives focus on enhancing outreach to those in institutions and reducing obstacles to making the transition. Services are available in Spanish, along with interpretation services.
