Comprehensive case management supports seniors in crafting a tailored care plan that addresses their physical, psychological, social, and financial requirements. Case Managers facilitate the coordination of essential services and monitor cases over the long term to guarantee that seniors obtain the necessary support. Additionally, Case Management services include help with completing paperwork for various programs. Services are available in English.
Intake Procedure: Call for information.
Fees: No Fee.
