Aim Independent Living Center - Chemung County Outreach Office

This initiative offers support to individuals with developmental disabilities, helping them access the resources necessary for leading fulfilling lives. It employs a person-centered approach that emphasizes the unique needs and preferences of each individual receiving services, aiming to enhance their overall quality of life. An Individualized Service Plan (ISP) is created to define the individual's goals, the methods for achieving them, and the support they will receive throughout the process. These plans are regularly assessed and revised to ensure they align with the individual's evolving needs. Additionally, the program provides assistance with various services, including future planning, residential arrangements, recreational activities, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental adjustments. Fees are established for these services.
