Family Services At 310 Pennsylvania Avenue

This program delivers care management for individuals dealing with serious mental health challenges, HIV/AIDS, or two or more chronic health conditions alongside significant behavioral, medical, and/or social risk factors, who require help in accessing services to stabilize and enhance their overall health. Services provided include outreach and engagement, thorough care management with assessment and planning, coordination of care and health promotion, comprehensive transitional care, support for clients and families, referrals to community and social support services, and the utilization of health information technology to facilitate service connections.

Fees: Clients incur no out-of-pocket costs.
