Care Management services cater to both Adults and Children with physical and/or mental health conditions. Care Managers help with setting up appointments, organizing transportation, and can accompany clients to appointments upon request to advocate for their needs and preferences. For children, a Care Manager collaborates with parents or guardians to ensure the child's safety, health, and overall well-being. They also coordinate with various providers, including schools, to ensure that the child's voice is acknowledged and that appropriate goals are established to address the child's requirements.
