This caregiver navigation program delivers essential support through crisis counseling, coordination of care, thorough needs assessments, connections to community resources, educational opportunities, advocacy with family or service providers, the creation of a caregiving plan, and ongoing follow-up. Caregivers are unpaid individuals, such as spouses, partners, family members, friends, or neighbors, who help another adult with daily living activities or medical tasks. The process involves the caregiver contacting 211, followed by intake and assessment, service planning and education, resource connection, follow-up, re-engagement, and ultimately, caregiver success. Services are available 24/7, with translation offered in over 180 languages. Intake is initiated by calling 211, and there are no fees for these services.
