This program facilitates the coordination of in-home health care and support services for families, overseen by their personal physician. Home care coordinators are available in all local hospitals, providing a range of nursing services. The program also includes rehabilitation and habilitative therapies such as occupational, physical, and speech therapy, along with certified hospice and palliative care. Additional services encompass home health care for those with developmental disabilities, social casework, mutual support groups for bereavement and caregivers, and volunteer opportunities.

Intake Procedure: Referrals for home nursing care evaluations must come through a physician, while homemaker-home health aide services can be accessed via agencies or self-referral.
