This initiative assists individuals with intellectual and developmental disabilities and their families in navigating available services and supports. It encourages informed decision-making and promotes independence. Care Coordinators are dedicated to linking individuals with appropriate care, support, and opportunities. They monitor circumstances and services during significant life changes, coordinate access to medical, dental, and behavioral health services, and ensure safety and residential needs are met. Additionally, they provide referrals to community and social support services and develop a comprehensive Life Plan that evolves with changing goals and needs.

Fees: Services are provided at no cost through OPWDD eligibility and Medicaid navigation, with Medicaid being a requirement for enrollment.

- Individuals diagnosed with intellectual and/or developmental disabilities before the age of 22, along with specific conditions such as autism spectrum disorders, cerebral palsy, and others, qualify for this program.
