This initiative supports individuals on their journey to recovery from mental health and substance use challenges who are ready to live more independently within the community. It offers a mix of onsite and scattered site apartments that are master leased in Monroe County, particularly focusing on Center City Rochester. Tailored services are provided based on individual needs and treatment plans, which include round-the-clock staff support, evaluation and treatment, medication management, counseling, independent living training, social and recreational activities, career counseling, family education, and coordination with other service providers, along with case management.
