This program offers authorization for services aimed at individuals who have experienced a traumatic brain injury or stroke. The waiver facilitates payment and budgeting for these services. Participants may qualify for rental assistance and housing support, as well as a one-time payment for furniture and household items. Additionally, it includes management of Medicaid funds and connections to various services.
Intake Process: Interested individuals should call for details. A referral form will be evaluated, followed by scheduling an interview.
