This program offers authorization for services aimed at individuals who have experienced a traumatic brain injury or stroke. The waiver facilitates payment and budgeting for these services. Participants may qualify for rental assistance and housing support, as well as a one-time payment for furniture and household items. Additionally, it includes management of Medicaid funds and connections to various services.

- Individuals must have a diagnosis of traumatic brain injury or stroke, be Medicaid recipients, and be between 18 to 64 years old, with eligibility for nursing home care based on a nursing evaluation.
