This program provides Children's Care Management for youth enrolled in Medicaid and Care Coordination services for those who are not. It aids young people with mental health diagnoses or trauma backgrounds in reaching their goals and connecting with essential resources.
Intake Procedure: Contact via email or phone.
Fees: Services are provided at no cost.
