About this resource
This program provides Children's Care Management for youth enrolled in Medicaid and Care Coordination services for those who are not. It aids young people with mental health diagnoses or trauma backgrounds in reaching their goals and connecting with essential resources.

Fees: Services are provided at no cost.
Who can use this
- Youth aged from birth to twenty-one with a mental health diagnosis or trauma history can access programs for both Medicaid and non-Medicaid participants.
