This initiative delivers in-home and community-based supportive services, such as therapy and rehabilitation, for children who are at risk of or have been formally diagnosed with a mental health condition. Services are accessible in Nassau and Suffolk for those who qualify for Medicaid.
Intake Procedure: Referral required.
Fees: No charge.
