This program focuses on case management for seniors facing behavioral health challenges. A dedicated team, which includes a registered nurse, a case counselor, and a peer support specialist, will conduct a thorough and confidential in-home assessment, create a personalized care plan, and deliver intensive short-term case management for individuals aged 55 and above who are dealing with behavioral health issues, substance use, or cognitive impairments that hinder their ability to access community services and resources. The program also aims to tackle any barriers that prevent these seniors from receiving necessary services.
