Citizens Inc. delivers Medicaid Service Coordination (MSC) for individuals with developmental disabilities. They collaborate with clients to develop an Individualized Service Plan (ISP) that thoroughly specifies all the necessary supports available to them. This plan is periodically reviewed and adjusted as required. The agency ensures continuous communication with all service providers, facilitates access to services and supports, advocates for the individual, and keeps a record of all coordination efforts.
