This program aids individuals in the application process for the TBI Waiver or NHTD Waiver and provides support in overseeing services to maintain quality, continuity, and a focus on the individual's needs. It includes assistance with coordinating waiver and community services for home safety, help with benefit paperwork, scheduling medical appointments, and finding resources that encourage independence.
