Health Home Care Management assists families in overseeing the health and wellness of children and youth aged 0-21 who are enrolled in Medicaid. This initiative fosters the well-being of children and families while minimizing the need for emergency interventions, absenteeism, and hospitalizations. The program aims to ensure that children obtain necessary care to thrive and that families feel empowered in managing their child's health.

- Children from birth to 21 years must have: 1) Active Medicaid 2) A diagnosis of serious emotional disturbance, HIV/AIDS, or mental health conditions.
