This program provides various services for both adults and children dealing with multiple chronic health issues, HIV/AIDS, or ongoing mental health conditions. Services include Care Management, Transitional care, support for patients and families, and referrals to community resources.

Fees: Covered by Medicaid and Medicaid Managed Care, with no enrollment fee.

- Participants must have at least two chronic medical or mental health conditions, or one qualifying condition, and must be enrolled in Medicaid. The Children's Health Home serves ages from birth to 18 years, while the Adult Health Home is for those aged 18 and above.
