This initiative supports individuals with disabilities in achieving an independent lifestyle through several resources, including Self-Direction for those with developmental disabilities, Medicaid Service Coordination, and Habilitation Services that aid with daily tasks and independent living skills. It also includes the Consumer Directed Personal Assistant Program, assistance for transitioning from nursing facilities to home (including moving help), respite vouchers for caregivers, and financial reimbursement for items that help individuals with disabilities stay in their homes.
Intake Procedure: Schedule an appointment by phone.
Fees: Services are covered by Medicaid, Medicare, or private pay, with a sliding scale based on income.
