This initiative assists individuals with intellectual and developmental disabilities, along with their families, in navigating available services and supports. It encourages informed choices and fosters independent decision-making. Care Coordinators are dedicated to linking individuals with the necessary care, support, and opportunities while monitoring circumstances and services throughout life transitions. They facilitate access to medical, dental, and behavioral health services, ensure safety and residential needs are met, and provide referrals to community and social support services. A comprehensive Life Plan is developed and adjusted as new goals and needs emerge.

Fees: Services are provided at no cost through OPWDD eligibility and Medicaid navigation, with Medicaid being a requirement for enrollment.

- Individuals diagnosed with an intellectual and/or developmental disability before the age of 22, along with significant impairments in social, communication, and daily living skills, as well as specific conditions like autism spectrum disorders and cerebral palsy, qualify for assistance.
