This initiative provides support to individuals with intellectual and developmental disabilities and their families, helping them navigate available services and resources. It encourages informed decision-making and independence. Care Coordinators are dedicated to linking individuals with appropriate care, support, and opportunities, while also monitoring their circumstances and services during life transitions. They facilitate access to medical, dental, and behavioral health services, ensure safety and residential needs are met, and provide referrals to community and social support services. Additionally, they develop a comprehensive Life Plan that evolves as new goals and needs emerge.

Fees: Services are provided at no cost through OPWDD eligibility and Medicaid navigation, with Medicaid being a requirement for enrollment.

- Eligible conditions include a diagnosis of intellectual and/or developmental disability before the age of 22, along with significant impairments in social, communication, and daily living skills, as well as specific disorders such as autism, cerebral palsy, epilepsy, and others.
