Wildwood Programs' Medicaid Service Coordination aids individuals with developmental disabilities in accessing a comprehensive range of community services and supports, while fostering the principles of choice, tailored services, and consumer satisfaction. The service coordinator collaborates with the individual, their family, and other service providers to create and execute a personalized service plan that reflects the individual's goals, aspirations, and dreams.
