This program focuses on the strengths of youth aged 0-21 who experience serious emotional issues, developmental disabilities (if they are in foster care), and medical fragility. Participation is voluntary, and families incur no costs. Staff engage with families and youth in their homes or other preferred locations.

Intake Procedure: Access to the Children's HCBS program is facilitated through a youth's Care Manager. If both the Care Manager and the family determine that the youth is likely to benefit from and qualify for these services, a referral is made to the program. Funding comes from State and Federal Medicaid, but children with private health insurance can also qualify through a different eligibility process.
