This initiative assists individuals with intellectual and developmental disabilities and their families in navigating available services and support systems. It encourages informed decision-making and independence. Care Coordinators are dedicated to linking individuals with essential care, support, and opportunities while monitoring their circumstances and services during life changes. They facilitate access to medical, dental, and behavioral health services, ensure safety and residential requirements are met, and provide referrals to community and social support services. Additionally, they develop a comprehensive Life Plan that evolves with the individual's goals and needs.

Fees: No cost through OPWDD eligibility and Medicaid navigation; enrollment requires Medicaid.

- Individuals diagnosed with intellectual and/or developmental disabilities before the age of 22, along with those having significant impairments in social, communication, and daily living skills, as well as specific conditions like autism spectrum disorders and cerebral palsy, qualify for the program.
