This program delivers case management and supportive counseling to individuals diagnosed with chronic diseases. Clients can utilize transportation to approved services and receive assistance in maintaining connections to care, as well as treatment adherence services, all integrated into a comprehensive HIV primary care Retention Adherence Program (RAP). Medical Case Management works in conjunction with these services to ensure clients can access the necessary resources for their health.
