This initiative aids individuals with developmental disabilities in obtaining the essential support and resources required for leading fulfilling lives. It employs a person-centered approach that prioritizes the individual's needs and aspirations while aiming for optimal quality of life. An Individualized Service Plan (ISP) is created, detailing the individual's goals, the methods to achieve them, and the support they will receive throughout the process. These plans are regularly assessed and adjusted as the individual makes progress to ensure that all services and assistance align with their needs. Additionally, it offers support for various services, including future planning, residential planning, recreational activities, residential habilitation, day habilitation, reimbursement options, adaptive equipment, and environmental modifications.
