Council On Aging Of Central Oregon
Community Resource
Acerca de este recurso
Options counselors support individuals in making knowledgeable long-term care choices by assessing their strengths, needs, preferences, and individual circumstances. They offer guidance on support strategies, action plans, and options available within the community. Options Counseling can serve as an independent service for participants or may transition into Older Americans Act Case Management (OAA), which is designed for those requiring ongoing assistance to maintain their independence. Care coordinators play a role in problem-solving, advocacy, and connecting participants with resources and community support to improve their quality of life and future planning for later years.
Intake Procedure: For more details, please call or visit the website.
Fees: There are no charges for this service.
