Options Counseling can serve as an independent service for participants or may transition into Older Americans Act Case Management (OAA), which is designed for those requiring ongoing assistance to maintain their independence. Care coordinators play a role in problem-solving, advocacy, and connecting participants with resources and community support to improve their quality of life and future planning for later years.
