iCircle Care delivers integrated health and social services to individuals aged eighteen (18) and above who are eligible for Medicaid and require more than 120 days of long-term care support due to chronic illness or disability, while meeting specific eligibility criteria. The program aims to empower members to live safely in their homes or communities. A Managed Long-Term Care (MLTC) plan facilitates the provision of long-term services to those who are chronically ill or disabled and desire to remain in their residences. Services such as home care and adult day care are offered through managed long-term care plans sanctioned by the NYS Department of Health.
