Children With Special Health Care Needs

This initiative offers care coordination for children, youth, and young adults statewide who have special healthcare requirements. It features two programs designed to assist these individuals and their families in accessing resources, services, and specialists that can improve their health and overall well-being. Additionally, it provides support for transitioning to adult healthcare and planning for adolescents and young adults. The services are delivered through the Integrated Services Program and the more comprehensive Intensive Care Coordination program.
