The Administrative Procedures Section (APS) is responsible for collecting public records in line with Idaho Public Law for the Department of Health and Welfare and supports the Department in formulating rules according to Idaho code 67-5201 et. seq., Administrative Procedures Act. State Street, 10th floor Boise, ID 83720-0036. To request a Public Record, the request must be written and include the requester's name, address, and phone number. It should specify details such as dates, locations, or names of reports relevant to the request. If the request pertains to a family member or client, a signed release from that individual is required. For client-specific information, fill out the Records Request form available at www.healthandwelfare.idaho.gov, under Public Records. If submitting the form to a local Health and Welfare office, bring identification. If sending by mail, fax, or email to PublicRecordsRequest@dhw.idaho.gov, your signature must be notarized. To obtain medical records in Idaho, request them from your healthcare provider or the Idaho Department of Health and Welfare. All public records requests must be in writing. Languages: English.
