About this resource
The Idaho Home Choice Money Follows the Person Demonstration Grant assists individuals in transitioning from institutions, such as skilled nursing facilities, intermediate care facilities, or mental health institutions, to community living options like apartments, private homes, or certified family homes and residential assisted living facilities. The MFP initiatives aim to enhance outreach to those in institutions and reduce obstacles to transitioning. Services are available in Spanish, along with interpretation services.
