About this resource
The Idaho Home Choice Money Follows the Person Demonstration Grant supports individuals transitioning from institutions, such as skilled nursing facilities, intermediate care facilities, or mental health institutions, to community living in settings like apartments, private homes, or certified family homes. The MFP initiatives aim to enhance outreach to those in institutions and reduce obstacles to transition. Services are available in Spanish, along with interpretation assistance.
