About this resource
The Idaho Home Choice Money Follows the Person Demonstration Grant supports individuals in transitioning from institutional environments, such as skilled nursing facilities, intermediate care facilities, or mental health institutions, to community living in apartments, private homes, or certified family homes, as well as residential assisted living facilities. The MFP initiatives aim to enhance outreach to those in institutions and reduce obstacles to transitioning. Services are available in Spanish, along with interpretation services.
