Our Path Home CONNECT functions as the gateway to the homeless services system, where each household facing homelessness undergoes an assessment and is placed on a prioritized list according to how long they have been homeless and the intensity of their service needs. Partner agencies within Our Path Home have committed to utilizing this prioritized list to assist our most at-risk community members, ensuring that all agencies receive referrals from this list to fill vacancies in their supportive housing programs.
