For replacement cards, dial 1-888-295-6850. USBank Child Support Services are designed for families where children live apart from one or both parents. These services can assist in: 1) Finding parents. 2) Establishing paternity (the legal bond between a father and child). 3) Creating or modifying a court order for child and/or medical support. 4) Collecting and documenting child support payments. 5) Enforcing child support responsibilities. A fee of $25.00 is required, with additional charges possible for specific services, such as legal assistance. The Division of Welfare oversees the Regional Child Support Program. For issues related to the Idaho Family Support Card, including replacement requests, card activation, PIN changes, balance inquiries, or transaction disputes, contact US Bank Customer Support. Services are available in English, with interpreters accessible upon request.
