Gentiva Health Services

This program is dedicated to assisting patients aged 65 and older in achieving their maximum health and autonomy by delivering specialized medical services directly to their homes. It includes a range of skilled healthcare services such as nursing, physical therapy, occupational therapy, speech therapy, social services, dietary consultations, and traditional in-home skilled care for medication management, wound care, and various disease and condition management, including education on diabetes, CHF, COPD, and urinary catheter care. Additionally, it provides four specialized services: Fall Prevention (Safe Strides Program), Orthopedics, Low Vision, and Neuro Rehabilitation. Services are covered by Medicare, Medicaid, and other health insurance providers. Language support is available in English.

Intake Procedure: The process begins with the submission of basic patient information via fax or phone.
