About this resource
Children who qualify for Medicaid and have a developmental disability may be able to receive home and community-based services via the Children's Developmental Disabilities (DD) Services Program. Applications can be submitted through mail (Childrens DD Application, DHW FACS 5th Floor, 450 West State Street, Boise), email (ChildrensDDIntake@dhw.idaho.gov), or fax (332-7331). Services are available in both English and Spanish.
