The program encompasses several health initiatives such as Living Well In Idaho, which is managed by Joanna Adams (Chronic Disease Self Management Programs, 208-415-5141), and I-PAN, led by Lisa Kolenda, RN (208-415-5284). It includes an Adolescent Pregnancy Prevention initiative aimed at high school students, and the Diabetes Education Program that collaborates with Idaho's health systems to mitigate diabetes and its complications through educational efforts. The program also offers the 'Control Your Diabetes' resource through Medicare, available at 1-877-214-5433, providing essential information for those affected by diabetes. Additionally, the Injury Prevention Program focuses on enhancing flexibility, strength, balance, and endurance for seniors aged 65 and above. The Oral Health Program, supported by the Maternal and Child Health Title V Block Grant, aims to promote oral health and prevent dental issues through local initiatives in partnership with the state's health departments. Lastly, the Tobacco Prevention, Control and Cessation Program works on reducing smoking and secondhand smoke exposure through education and advocacy, while also providing free cessation classes for pregnant women, youth, and adults, supported by the Idaho Tobacco Millennium Funds.
