Application Process: To apply, you can call for assistance or request an application to be sent to you. If you prefer to apply online, be sure to print and submit both the Statement of Treatment form and the Income Statement. Aid is allocated on a first-come, first-served basis, depending on available funding. To find out if your condition is among those supported by the HealthWell Foundation, please call or visit the website, as some diseases may no longer accept new applicants.
